CONTRACT
Before me, the undersigned Notary and in the presence of the two undersigned witnesses,
PERSONALLY CAME AND APPEARED:__________________________________

a person of full age and majority and a resident of the state of Louisiana and
the CITY OF OPELOUSAS, Louisiana,
a municipality represented herein by its Mayor,___________________________________, duly authorized
by the Board of Alderman, who declared that, in consideration of their mutual promises they have entered
into the following agreement:
That the said,__________________________________, upon
his/her hiring by the City of Opelousas
as a police officer agrees to reimburse the City of Opelousas for all the cost for basic training, cost of his/her equipment,
cost of his/her uniform and the cost of any additional training if he/she terminates his/her employment with the
Opelousas Police Department within two years from the date of hire.
This instrument contains the entire agreement between the
parties hereto. There are no promises, terms,
conditions or obligations other than those stipulated and
contained herein.
